SA Heart Patient Identification Card introduced

SA Heart has recently introduced a Patient Identification Card. This convenient ‘credit card’ size card, suitable for storing in your wallet or purse, is available to all SA Heart patients upon request at any SA Heart clinic.

If you need to attend an emergency department anywhere in Australia, you should present your card to the Emergency Department admitting staff and request that SA Heart be immediately notified of your arrival.

By promptly contacting the treating SA Heart cardiologist or the on call, alternative SA Heart cardiologist after hours, the emergency department will gain prompt access to your secure, electronically stored patient record to provide you with optimal continuity of care.

If you would like a Patient Identification Card, please ask our friendly reception staff or your cardiologist to provide one for you when you next visit an SA Heart clinic.
